MEMBER TOTAL SYSTEM INSPECTION Chris. 5/8/24 STELLAR PERFORMANCE MEMBERSHIP COOLING SEASON INSPECTION. Performed gas leak test—pass. Checked thermostats—pass. Checked furnace air filter & blower compartment—pass. (Furnace blower wheel needs to be cleaned.) Checked water heater tank and operation—pass. Tested the water for hardness and total dissolved solids— 0.0 hardness A mouse nest was found in the wiring compartment on the air conditioner. This had damaged the high-voltage switch, and they chewed at least one wire leading to the compressor. I could not perform refrigerant pressure testing. I have pulled the AC disconnect until homeowner reviews estimates. Provided options for correcting deficiencies. I will also schedule a second trip for comfort advisor to quote new air conditioner install and recheck undiluted CO in the furnace. 3/4/24 JOSEPH FURNACE IS SHORT CYCLING Unit was shutting down on high limit. Check manifold gas pressures. They were ok. Determined that the filter is too restrictive. Recommended polarized option. 2/28/24 JOSEPH & CHRIS NO HEAT Spent some diagnostics time finding a loose wire on the 24 volt secondary circuit. Corrected it and the unit is now running. Beena asked us to install their NEST thermostat. We installed it and also replaced some bad control wiring at the furnace. Set up the thermostat to work with the exist equipment. Shop Boiler Replacement. Turned off the power to the existing boiler. Shut off the gas and removed all electrical connections from the existing boiler. Remove the system piping, gas piping, drain piping, and vent piping from the existing boiler. Removed the existing boiler from the wall and mounted the new boiler on the wall. Connected the vent piping, gas piping, drain piping and electrical connections. Reconnected the system piping relocating the circulation pump at the same time. This will provide better system operation and reliability. Refilled the system with antifreeze solution. Adjusted gas pressures and tuned up the boiler for proper operation. The water heater, and water softener are operating properly at the completion of the inspection. 01/03/23 JOSEPH Check the heat exchanger. Did not see any verifiable cracks. There were some deep scratches that look like they could be cracks but after examining from multiple angles, determined that these were not cracks. Verified gas pressures. Logan Chris and Lance Replaced the furnace. Installed a new Goodman 98% furnace. The furnace was running properly after the install was completed. We ran a combustion test on the new furnace and it passed perfectly. No issues with new install

12/21/23 JOSEPH, LOGAN & CHRIS INSTALL CUSTOMER PROVIDED MINI-SPLIT Wall mounting bracket was installed by customer before our arrival. Installed the line set, drain line, and electrical wire. Connected them to the indoor unit. Hung the indoor unit on the wall mount bracket. Installed wall brackets outdoors. Set the outdoor unit on the brackets and secured them with bolts. Connected the line set and electrical to the new outdoor unit. Ran the drain line to disposal point. Secured the line set, electrical and drain line to each other with zip ties. Charged the system with nitrogen as a holding charge until we are able to return to complete the work. Received final payment in full since the customer will be out of town when we complete the work. 12/28/23 JOSEPH COMPLETE THE INSTALLATION Released the nitrogen charge. Evacuated the system with vacuum pump. Charge system with refrigerant. Started up the system to verify proper operation. The system is operating properly at the completion

12/28/23 JOSEPH ANNUAL MEMBERSHIP BOILER INSPECTION Performed boiler inspection. Found defective heat exchanger on the boiler. Provided options on replacement. Also, John and Luann enrolled in the membership plan for the house. We checked the heat exchanger on the furnace and found 11 sections of the heat exchanger were corroded through. Provided a furnace replacement option. We are going to replace the boiler and the furnace. Received separate checks for initial investment on each piece of equipment.
